Release and Waiver of Claims:

The undersigning participant of Kenpo Karate or Cardio Kickboxing hereby releases Grass Valley Kenpo Family Karate, its agents and instructors, and students from liability as a result of any injury, known or unknown, through negligence or not, suffered by the participant while involved in the martial arts. The participant and/or guardian acknowledge that certain element of training are physically demanding and he or she participates in the training voluntarily with fully knowledge of the risk involved in physical exercise.

The participant and/or guardian signing this acknowledge that they should have received permission from a medical doctor following a general physical examination approving the participant’s involvement in these instructional services. The participant and/or guardian also agree to be in good physical condition and is able and allowed to participate in exercise and various martial arts curriculum which is provided by Grass Valley Kenpo Family Karate. The participant and/or guardian understand that due to the nature of the training which is being provided by Grass Valley Kenpo Family Karate, accidents do from time to time occur. While all precautions will be taken to assure the safety of all participants, it is impossible to guarantee that such accidents will not occur. In the event that the participant is involved in an accident which subjects him or her to any injury, the participant and/or guardian agrees to hold Grass Valley Kenpo Family Karate and/or its instructors harmless; you further agree that neither the participant, parent, guardian, nor any other financially responsible person will take action against aforementioned parties.

This release is intended by the participant and/or guardian and Grass Valley Kenpo Family Karate to release all claims of liability, injury, damage, or losses to the participant‘s person or property, through negligence or not, and both parties acknowledge the significance and consequence of such specific intention to release all claims. This release is freely and voluntarily executed by the participant and/or guardian with full knowledge of the risk of injury. The participant and/or guardian further acknowledges that he or she has read this release and does not rely on any inducements, promises, or representations made by Grass Valley Kenpo Family Karate or its agents.

Grass Valley Kenpo Family Karate Policies

Monthly dues MUST be paid by the 1st of every month. A $10 late fee will be applied after the 15th. Students are required to pay their dues at the time of sign up. The fee for bounced checks is $27. If more than one check is bounced we will no longer accept your checks. Testing fees are to be paid the day of the scheduled event.

Students of Kenpo must come prepared to EVERY class with a mouthpiece and groin protection. If they do not bring this basic personal protection to every class, they may be asked to sit out during that day’s practice. While we do not require purchasing sparring gear, we highly recommend it to ensure proper fit and maximize safety.
You do not have to purchase a gi (uniform) right away. But the gi is required before the Yellow Belt test for ages 9 and up.

Custom orders and private lessons must be pre-paid. No refunds.

In accordance with Kenpo principles, it is our responsibility to refuse service to anyone who abuses their martial arts training. We also reserve the right to refuse service to anyone who exhibits conduct or attitude detrimental to themselves, their classmates, community, or our classroom environment. Only students who follow our guidelines and tenets, including treating their instructors and peers with dignity and respect, will be allowed to train at Grass Valley Kenpo Family Karate. Determination of eligibility is at the discretion of Mr. and Mrs. Garcia.

Visiting martial artists may come and join any regular class with the permission of Mr. and Mrs. Garcia. In order to participate, you must provide a picture ID and sign our waver.

Lastly, you agree to sign a hard copy of this form and the Covid-19 waiver in the dojo.

This agreement is between the student/parent and Grass Valley Kenpo Family Karate.
